2021 COVID-19 Business Support Grant Supporting Documentation Checklist 

PLEASE ENSURE YOU HAVE PROVIDED DOCUMENTS FROM EACH SECTION TO SUPPORT YOUR APPLICATION.

Reduction in Turnover of 30% or more over 7-day period 

  • Declaration in application (Directly - closed due to a government health direction businesses) 
  • Sales Turnover information for the nominated 7-day period and the corresponding period from 2019 (Indirectly impacted businesses) 

E.g. Profit and loss statement from accounting software or other sales records for the 7-day period for each year 

 

Employ Staff 

  • Business financial statements 
  • Payroll records E.g. Payroll summary report from accounting software, or 
  • Payroll tax information E.g. monthly payroll tax return lodgement 

 

Annual Payroll under $10 million during 2019, 2020 or 2021 

  • Payroll records E.g. Payroll summary report from internal accounting software, or 
  • Payroll tax information E.g. payroll tax annual return 

 

Annual Turnover over $75,000 during 2019, 2020 or 2021 

  • Income Tax Return, or 
  • BAS Statements, or 
  • Income reporting reasonably showing you expect to hit $75,000

 

Evidence of Trading Location 

  • Publicly reachable web information E.g. website or social media page with listed business address; ABN advice 
  • Utility bills for the business location E.g. electricity invoices or rates/water notices 


Bank account 

Nominated bank account to receive payment to.
